This program offers personalized nutrition content advice to help manage your nutritional needs and goals. A powerful combination of meal tracking and personalized support from a registered dietitian will help you gain effective tools and expert support to improve your nutrition practices.

On the
Care screen, select
Total Health and Life Services and then Personalized Nutrition.

Review the program description and select Service Request.

On the Introduction page, select Start.

Select which of the categories you would like help with on the Category page, and select Next.

Enter any relevant information you want to share with your assessment nurse on the Additional Details page, then select Next.

On the Payment Method on File page, enter your credit card details, and select Submit.

You will skip this step if you already have a payment method set up. We will not charge your credit card during this step of the process, but we do require one in order to charge you for future transactions.

From the Navigation Bar, select Care Services and then
Total Health and Life Services and then Personalized Nutrition.

Once you submit a request for the Personalized Nutrition program, you will be matched with an assessment nurse to book an initial assessment at no cost.

At the assessment, your nurse will discuss the details of the Personalized Nutrition program and enroll you if you would like to continue with it.

Once enrolled in the program, you will have access to RxFood and be matched with a registered dietitian who will provide three hours of health coaching.

Before meeting with your registered dietitian, complete up to three days worth of meal tracking on RxFood to create a robust picture of your everyday diet.
Caution
If meal tracking is sensitive or triggering for you, please discuss this in your initial assessment, and your nurse can discuss alternative options for you.

When you are ready to meet with your nutrition specialist, follow the steps in the Book an appointment with a Total Health and Life Services care provider article to book your session. Your dietitian will create your personalized health plan using information you enter into RxFood and your health coaching sessions.
