Why was my dental exam denied due to frequency?

There are various exam codes that a dentist might perform and some of these procedures share the same frequency which might affect eligibility. 

We recommend that you verify your coverage prior to having any dental work done. You can have your dentist submit an estimate for the recommended services. When we have assessed the estimate we will produce a written response outlining the amount that will be covered by your plan.
