View personalized claim forms

Instructions for the mobile app

1. On the  Home page, select View personalized claim forms.

2. Choose the claim form you wish to personalize, then select Continue.

3. Choose who the form is for then select Download claim form.

Instructions for the web app

1. From the Dashboard, in the Do More with your claims section, select Personalize my claim forms.

2. Select the type of form you would like to personalize and the person whom the form applies to.

3. Select Get Form.
