Instructions for the mobile app

1. On the Care screen, select Telemedicine and then Telemedicine.

2. Select Settings.

3. Select Deactivate Telemedicine Account.

4. Select a reason for deactivating.

5. Select Deactivate Telemedicine Account.

Instructions for the web app

1. From the Navigation Bar, select Care Services and then select Telemedicine.

2. On the Telemedicine Dashboard, select Telemedicine Settings.

3. Select Deactivate Account.

4. Select a reason for deactivating.

5. Select Close account and then select Close account.
