Our matching questionnaire was designed to help you find a mental health counsellor that can address your specific needs. You are encouraged to prioritize your wellbeing and take the time you need to complete the questionnaire, and you have the option to save and come back later at any point.
Note
Please note that you will not be able to match with the same counsellor for individual and couples counselling.

On the
Care screen, navigate to
Mental Health and then Counselling.

On the Counselling Overview screen, navigate to the Services section, select for which service you would like to find a counsellor, then select Match with a Counsellor.

On the Choose your path screen, select Start Questionnaire.

For each question in the Wellbeing Assessment section, slide the cursor on the ranking scale to indicate how you are feeling, then select Next.

In the Self Evaluation section, select the option(s) that best represents your feelings with regard to each question, then select Next.

In the Diagnosis and Stressors section, select any of the options that apply to you for each question, then select Next. If an applicable answer doesn't appear in the list, you can select Other and specify.

You will also be asked to select whether you spend most of your time at work, at school, both or neither, and to indicate any distress factors in the selected environment(s).

In the Personal Details section, verify your Date of birth, your Province or territory of residence, and your Postal code, then select Next.

In the Counsellor Preferences section, select your language, specialties, and religious or spirituality expertise preferences, then select Next.

On the General Availability screen, select your availability and select Next.

Scroll through the list of counsellors to see your potential matches. When you've found a counsellor you would like to match with, select Match with Counsellor.

To view more information about a counsellor in the list, select View Profile.

On the next screen, select Confirm Match.
Your mental health counsellor will now appear on the Counselling Overview screen and on the Services screen.

On the Navigation Bar, select Care Services, then select
Mental Health and Counselling.

On the
Counselling dashboard, navigate to the Services section, select the service for which you would like to find a counsellor, then select Match with a counsellor.

On the Choose your path screen, select Start Questionnaire.

For each question in the Wellbeing Assessment section, select the number on the ranking scale that best represents how you've been feeling (1 being very poorly and 10 being very well) and select Next.

Your mental health counsellor will now appear on the Counselling dashboard in the My care team section.
